Application

To get started the first thing you need to do is to fill out our online initial application to see if you qualify to become a gestational carrier.

We will review your application and contact you within two business days to let you know if you are accepted into our program.

Profile

The next step in the process will be to fill out the surrogate questionnaire. The answers you give us on the questionnaire will enable us to get to know you and your preferences better which will help us find the right match for you.

Once we have received your questionnaire responses we will schedule a Zoom call with you to discuss your matching preferences and answer any questions you may have.

Matching & Meeting

Our intended parents fill out a very similar questionnaire. We will compare your responses with those of our intended parents. When we find an intended parent we think will be a great fit, we will send you their your profile. If you like them, we will share your profile with the intended parents. If they agree you look like a good fit, PrimaVita will set up a meeting between you and the Intended Parents.

After match call, PrimaVita will connect with both parties to find out if everyone would like to move forward with one another. If all parties agree, we will share contact information. We want you to start bonding right away!

Medical Verification
  • PrimaVita will have your pregnancy/delivery records sent to your intended parents’ fertility clinic.
  • PrimaVita will initiate insurance verification to find out whether your insurance will cover a surrogate pregnancy.
  • PrimaVita will coordinate psychological evaluations for both the you and your intended parents, based on clinic requirements.
  • PrimaVita will schedule medical screening for you at the fertility clinic.
Surrogate Process

Tips for your first Zoom call with potential IPs

  • Try to find a quiet place where you won’t be interrupted. Giving your full attention to the call will show your IPs this is important to you.
  • For a Zoom call, it’s a good idea to open up Zoom before your scheduled call and give it a quick test to make sure everything is working correctly.
  • Review your IPs profile again and write down questions you want to cover during your call. It’s easy to forget things once the conversation starts flowing.
  • Don’t stress… try to relax! Your IPs are likely just as nervous as you are! After your phone call is over, call or email your coordinator to let her know how things went.

Here are some sample questions or topics you can ask to keep the conversation flowing:

  • What led your potential IPs to surrogacy?
  • What are your expectations for the birth?
  • What are your hopes and expectations for communication? (frequency and what form of communication they prefer)
  • What kind of relationship do you expect from me? (before, during and after the birth)
  • How do you feel about multiples?
  • Ask general questions about family and lifestyle.
  • Are you on board with meeting my children and allowing my children to meet the baby after he/she is born?
  • Do you plan to tell your child about surrogacy when he/she is older?
  • Does your family know about your plans for surrogacy? Are they supportive?
  • How do you spend your free time?
  • Who will be present during the birth?
  • What are your thoughts on termination? Under what circumstances do you feel that it is necessary?
  • Do you want me to provide you pumped breast milk after the baby is born?
  • How soon are you looking to transfer?
  • Are you using your own eggs/sperm or donor?
  • How many children are you hoping to have via surrogacy? Are you open to a sibling journey if all goes well?
  • What kind of relationship can I expect from you after the birth of the baby?
  • If you live internationally, how do you plan to be involved in this journey? (Zoom/FaceTime, visits, etc.)
